NOTE: All Word doc must be saved to your computer and then sent via e-mail as an attachment. If doc is not saved all items entered will be lost.

Click on file > Save file to your computer > Open file > Fill out information > Save file again > send it back to us as an attachment in your e-mail.

DO NOT open from its location and send as e-mail through your internet explorer. All your information will be lost!
